Name: This is where you put your character's name. Any nationality is fine.
Age: Unlike DDT, the age can be literally anything. Just remember to accept whatever perks and consequences come that age.
Gender: This should be easy to figure out.
D-Phone Color: The D-Phone is based on the "Katana" cellphone. Here's a sketch of the D-Phone for you to figure out your color(s).
Note: This is the default Digivice that appears when a Contract is formed.
For Daedalus, their D-Phone becomes an Oracle.
For Crowns, their D-Phone becomes a Crown Phone.
For Stability, their D-Phone becomes a Grimoire Phone.
Physical Description: This includes skin tone, eye color, hair color, build, and height, as well as any deformities. For females this can also include bust size, but that isn't necessary.
Personality: The personality of the character. This can tell if the person is a true blue hero, an evil bastard, or anything in between and beyond, so try and be descriptive with this.
Clothing Description: This can be one set of clothing, several, or a general idea of the kind of clothes people wear. A character can't always wear the same school uniform or shirt.
Wish: If your character has a Digimon partner, this is the wish they wanted granted to gain their support.
Digital Enhancement: Once again, only for those with Digimon partners. If you do have one, this is the enhancement done to the character's body to help protect them. The powers are not in the same extravagant range as in DDT, so think conservative, not god-like.
Background: The background information for your character, as well as where you'll put any updates to your character's life as you go along.
Occupation: What the person does, from being a student to being a teacher. They can have as many jobs as they can humanly handle.
Personal Information: This is where you put various information like friends, personal enemies, favorite things, hated things, any vehicles they own, personal skills, weapons, etc.
Digimon Partner: This is also easy to figure out.

Attribute: The attribute is Virus, Vaccine, and Data. That's it. Though an "attribute" doesn't guarantee defeat or success, it can be an extra advantage, and allows for certain upgrades the other attributes can't get.
Physical Description: Same as the human partner, what the Digimon at Child/Rookie level looks like.
Personality: The Digimon's base personality. Try and be descriptive.
Live Disguise: The disguise of the Digimon, based on their height and general size. They can disguise themselves as people, vehicles, objects, whatever, as long as it makes sense. Walking around with a super model or a giant robot brings a lot of unwanted attention.
Attacks: List the attacks of the Digimon and describe them.
Final Charge: The Digimon's Final Charge attack. Same goes as with the "Attacks".
Digimon Evolution Table: This is where the Digimon's evolutions are done up. The highest level a Digimon can enter in as is Adult/Champion, however if you do this they will not be a full powered adult, and must go through a task to regain that lost strength.
The Digimon's evolutions are split between 3 paths, which can be switched out at any time once your D-Phone gets the upgrade to use it. The paths are completely different from one another, only starting with the same Child level.
Path A: Child => Adult => Perfect => Ultimate
Path B: Child => Adult => Perfect => Ultimate
Path C: Child => Adult => Perfect => Ultimate
